Welcome to your Sleep Assessment
This sleep apnea questionnaire is a proven tool used by sleep experts and board-certified physicians to screen for obstructive sleep apnea. Questions are focused on gender, age, observed apnea, tiredness, blood pressure, body mass index (BMI), neck circumference, and snoring. Answering these questions can help identify patients at risk.
If you answer yes to 4 or more questions, you are considered at high-risk for OSA. If you answer 3 or fewer questions with a yes, you are considered low-risk for OSA.
Dr. Cannon will follow up on the risk assessment results by using a variety of measures, such as home sleep apnea testing, sleep history, and self-reported signs to determine whether you suffer from a sleep disorder.
